Job Responsibilities:
• Identify, plan, coordinate community-based services for Clients.
• in-service training for CNA/Aides
• Conduct in-home visits and assessments.
• Maintain Client records.
• Conduct skilled service for clients in home:
Wound care/ dressing changes
Skin Monitoring
Tube Feeding
Ostomy and catheter care
Medication Management
Insulin/ syringe management
Bowel Management
Client/ Family training
Qualifications:
• At least 3 years’ experience in the health and human services field
• Active RN license, with at least 3 years of experience in Home Health Care
• Experience in social work, home and community-based services, healthcare or geriatrics preferred
• Good communication and customer service skills
• Excellent organizational and decision-making
• Valid Driver’s License
• CPR/First Aid
• Current TB
