What You’ll Do

As a Nurse Care Coordinator, you’ll play a critical role in ensuring seamless communication, flawless organization, and consistent high-quality care for our clients and caregivers.


Your key responsibilities include:

Client & Team Support

  • Serve as a trusted, professional point of contact for staff, clients, and families
  • Personally welcome new clients with a care call and home visit
  • Handle questions with poise, professionalism, and empathy
  • Maintain proactive communication through regular service check-ins
  • Coordinate with other medical professionals to ensure seamless support for our clients
  • Maintain accurate documentation of requests, client changes, and actions taken to support clients and the team

Training & Guidance

  • Provide support and guidance for new caregivers and nurses, ensuring each care team you work with is equipped to support quality of life for our seniors
  • Organize and track coaching and training opportunities within care teams
  • Support the client's circle of care with education and resources
  • Provide guidance to staff on policy and procedures, upholding our high standards of quality care

Field Visits & Quality Checks

  • Conduct in-home visits, care assessments, and quality assurance checks across Northern Michigan
  • Ensure care delivery aligns with client expectations and organizational standards
  • Facilitate team meetings to ensure consistent delivery

What We’re Looking For

  • Proven background in office administration (Healthcare, senior care, medical office, or service coordination experience highly valued)
  • Preference given to those with nursing administration experience
  • Exceptional communicator—professional, empathetic, and clear in all interactions
  • Highly organized and detail-oriented—able to manage multiple priorities with precision
  • Strong problem-solving and leadership skills—proactive in identifying needs and creating solutions
  • Proficiency with Microsoft 365 (Outlook, Teams, Word, Excel) and adaptability with new software
  • Valid driver’s license, insured vehicle, and ability to pass a background check
