What You’ll Do

As a Care Coordinator, you’ll manage and support a dynamic team of caregivers, create client schedules, and ensure the highest standard of care for every client we serve.


Your key responsibilities include:

Client & Caregiver Support

  • Personally welcome new clients with a care call and home visit
  • Serve as a trusted point of contact for caregivers, clients and their families
  • Address concerns with professionalism and empathy

Scheduling with Purpose

  • Create schedules that align client needs with caregiver strengths
  • Support care teams to provide high quality care

Training & Onboarding

  • Create warm, supportive relationships with new caregivers
  • Identify and coordinate ongoing coaching and training opportunities within care teams

Field Visits & Quality Checks

  • Travel to client homes across Northern Michigan for in-home visits, care assessments, and quality assurance

What We’re Looking For

  • Experience in healthcare, home care, hospice, or social work strongly preferred
  • A passion for working with seniors and supporting caregiving professionals
  • Strong communication and organizational skills
  • Proficiency with Microsoft 365 and ability to quickly learn new technology
  • Valid driver’s license, reliable insured vehicle, and clean background check

A Bachelor’s degree is preferred, but equivalent professional experience is also welcome!
