What You’ll Do

As a Care Coordinator, you’ll manage and support a dynamic team of caregivers, create client schedules, and ensure the highest standard of care for every client we serve.

Your key responsibilities include:

Client & Caregiver Support

Personally welcome new clients with a care call and home visit
Serve as a trusted point of contact for caregivers, clients and their families
Address concerns with professionalism and empathy

Scheduling with Purpose

Create schedules that align client needs with caregiver strengths
Support care teams to provide high quality care

Training & Onboarding

Create warm, supportive relationships with new caregivers
Identify and coordinate ongoing coaching and training opportunities within care teams

Field Visits & Quality Checks

Travel to client homes across Northern Michigan for in-home visits, care assessments, and quality assurance

What We’re Looking For

Experience in healthcare, home care, hospice, or social work strongly preferred
A passion for working with seniors and supporting caregiving professionals
Strong communication and organizational skills
Proficiency with Microsoft 365 and ability to quickly learn new technology
Valid driver’s license, reliable insured vehicle, and clean background check

A Bachelor’s degree is preferred, but equivalent professional experience is also welcome!
