We are very selectively looking for a Scheduling Care Coordinator to join our professional team in
Sumter and surrounding areas.
In this role you will:
- Ensure clients receive optimized levels of care to ensure favorable customer satisfaction
- Communicate and coordinate care plans, adjustments, and special requests to all stakeholders
- Scheduling of team to ensure continuity of care and adherence to plan
- Motivation of team to achieve stated organizational goals and outcomes
