Job Overview:

This role involves direct face-to-face interactions with clients and their families, helping to ensure that they understand and are satisfied with our services. Primary responsibilities are to answer leads, schedule and perform assessments and track the client journey to ensure satisfaction.
Key Responsibilities:
• Visits prospective clients after referrals are made to introduce Comfort Keepers.
• Establishes initial client records and maintains client compliance by setting tasks and follow up.
• Visits clients to keep abreast of client's condition and environment on a bi-weekly to monthly basis
• Responsible for determining type and duration of services necessary to meet client needs. Quotes bill rate based
• on service provided
• Assist in matching caregiver qualifications and availability to client’s needs.
• Supervises caregivers and completes performance appraisals for caregivers at specified intervals.
• Contacts clients and caregivers for follow-up on service delivery and determine satisfaction of services.
• Serves as a liaison between the client, caregiver, and scheduler.
• May serve on committees and participate in the development of systems to improve care coordination.
• Participates in on-call rotation as assigned. Participates in client conferences as requested by supervisor.
• May interface with other community resources to assure that client needs are adequately addressed.
